Research has shown that inadequate rest and recovery can lead to decreases in testosterone levels. As a result, it is important for weightlifters to carefully manage their training intensity and volume to avoid overtraining and potential negative effects on testosterone production. Furthermore, weightlifting has been found to have a long-term impact on testosterone levels. The relationship between testosterone and muscle growth is therefore bidirectional, with testosterone promoting muscle growth and weightlifting increasing testosterone levels. There were no differences in baseline characteristics of study completers and drop-outs (10). The predominant cause of drop-out was unwillingness to complete all required study procedures. The details on study design as well as the results on insulin sensitivity, inflammation and body composition from that trial have recently been published (10). The data below are a secondary analysis of a trial that evaluated the effect of testosterone replacement therapy on insulin sensitivity in men with HH and type 2 diabetes (10). Astudy of 14 eugonadal, β-thalassemic men found 1 to be azoospermic, 3oligozoospermic, 5 asthenozoospermic and 2 teratozoospermic. Additionally, synthesis of the iron-sulfur (heme) clusters thatform the catalytic core of iron-containing proteins occurs in the mitochondria andis affected by mutations in the mitoferrin genes . Iron may also play an important role in the mitochondria, as mutations inmitoferrin (which transports iron across the inner mitochondrial membrane) resultsin sterility in male fruit flies, with 100% penetrance . There is close overlap between these supplemental iron concentrationsthe normal range of seminal plasma iron concentrations (1.0 to 3.7 μg/ml). The need for iron in male fertility is underscored by the observation thatseminal plasma is the only known bodily fluid where iron is actively secreted.Transferrin is one of the most abundant proteins in seminal plasma, comprising up to5% of the total protein . It may be difficult todetermine whether altered semen parameters are directly due to iron overload,reactive oxygen species, hypogonadism, or a combination of these factors. In essence,testosterone supplementation (particularly high dosage use/abuse), physiologicallycauses hemochromatosis. A small study of men with transfusion-dependentβ-thalassemia, however, reported elevated seminal plasma iron concentrationsin five of six men, with concentrations approximately 5–10 times higher thanthe reference range . Seminal plasma iron concentrations are reportedly between 1.0 and 3.7μg/ml in healthy, normozoospermic men . Thus, some of the side effects of supplemental testosteroneuse may be directly related to iron overload. Thus, testosterone’s abilityto override the regulatory mechanisms to maximize iron absorption would beneficiallyincrease iron stores needed for rapid growth. The data are not available for various indices at week 24 and hence have not been presented. Studies have also shown that obesity is inversely related to sperm counts, sperm morphology and sperm motility(16, 17). These abnormalities are independent of the duration and severity of hyperglycemia but are directly related to insulin resistance. Iron impacts all aspects of men’s reproductive health, although notnecessarily in a negative manner. Nonetheless, it appears that severe iron overloadmay also negatively impact ejaculatory function.
